Job Description :

We are seeking a dedicated Physical Therapist to join our home health team. This role focuses on providing personalized therapy services in patients' homes to improve mobility, strength, and overall physical function.

Responsibilities :

Assess patients' conditions and medical histories

Develop individualized rehabilitation treatment plans

Conduct therapeutic exercises to improve mobility and function

Educate patients on injury prevention, recovery techniques, and home exercises

Monitor patient progress and adjust treatment plans as needed

Collaborate with healthcare professionals for coordinated care

Maintain accurate patient records and documentation

Qualifications :

Master's degree in Physical Therapy

Licensed Physical Therapist in the state of Pennsylvania

At least 1 year of experience in home health or related setting

Current CPR certification

Valid driver's license and ability to travel to patient locations

Strong interpersonal and communication skills

Patient-focused and compassionate approach
