Description : RN Case Manager, ED

The RN Case Manager in collaboration with other internal and external disciplines directs and coordinates the care of patients from admission to transfer or discharge. This is done with the intentions of cost effective utilization of hospital resources, minimizing out of pocket expenses for patients, providing customer support to our internal and external customers, appropriate utilization of resources and patient advocacy.

This position will work in the ED. 4 , 10 hour shifts with an every other weekend rotation.

Minimum Requirements

Education : RN Case Manager : Graduate from a school of nursing.

Baccalaureate degree in nursing preferred

Licensure / Certification : Current license to practice as registered nurse in the State of Ohio

Experience : Minimum of 3-5 years recent clinical experience in an acute care setting. Discharge planning or case management experience preferred.

Effective Communication Skills

Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.

Possesses excellent clinical assessment and analytical skills.

computer competency

Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations.

Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care
