Pay : $2,325 / week

Job Description

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Lancaster, NH, on a travel contract. The Physical Therapist will evaluate residents, create personalized treatment plans, and provide therapy services aimed at restoring mobility, enhancing strength, and promoting independence. The ideal candidate is patient-centered, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities

• Conduct evaluations to assess residents’ functional abilities, mobility, and therapy needs.
• Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
• Provide therapeutic interventions including gait training, balance activities, strengthening, and mobility support.
• Educate residents, families, and caregivers on therapy goals, safety strategies, and the use of assistive devices.
• Document evaluations, treatment progress, and outcomes accurately in compliance with facility and regulatory standards.
• Collaborate with nursing and therapy staff to deliver coordinated, patient-centered care.
• Support resident independence, safety, and quality of life through evidence-based practices.
