Requirements
- Required : must have actual state license and DEA in hand, Must be board certified
Active State Medical License, CDS, & DEA (with TX address), Board Certified in ABEM OR ABFM OR ABIM (or osteopathic counterparts),
Must be residency trained in Specialty Certified in ACLS, ATLS, & PALS, CME past 2 years, case logs past 2 years, copies of all education & training diplomas, SS Card, Driver's license, color photo emailed to MSO, TB / FLU, Annually must have 9 hrs of Stroke CME and 9 hrs of Trauma CME. Current ATLS not required for ABEM providers & only need to provide copy of initial ATLS.Schedule
