The Advanced Practice Provider (APP) works within a clinic as part of a collaborative team, which may include Family Practice and / or Internal Medicine Physicians, RN Case Managers, and clinic staff. This team operates under a comprehensive, coordinated care model to ensure high-quality patient outcomes.
The APP primarily manages the patient population in collaboration with the primary care physician and / or specialist. Depending on the group's needs, the APP may provide care in outpatient and / or inpatient settings, though most positions are outpatient unless otherwise specified. Acting as an extension of the primary care physician, the APP delivers both chronic and acute care services to patients.
Performance metrics tied to the organization’s strategic goals and operational requirements will be used to monitor and evaluate the APP's effectiveness and outcomes.

Requirements :
The Advanced Practice Provider (APP) delivers patient care in accordance with their licensure and certification.
Diagnoses and manages both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic illnesses, as well as minor injuries and acute medical issues such as infections or exacerbations of chronic diseases.Orders and interprets diagnostic tests, including lab work, x-rays, and EKGs.Prescribes medications and other treatments as necessary to address identified conditions.Educates patients about their medical conditions, treatment plans, and expected outcomes to ensure they are well-informed and engaged in their care.Collaborates as part of a comprehensive, multidisciplinary team to provide high-quality, evidence-based care.Performs additional duties as assigned to support patient care and organizational goals.Minimum Required Education, Experience & Skills
Graduate credit for PA / NP ProgramCurrent Licensure through the Georgia BoardCurrent prescriptive authority from the Georgia BoardKey physical and mental requirements :
