The Nurse Care Manager in DD Healthcare Home is part of the person-centered integrated care team and provides individual and population-based health interventions to persons enrolled in the DD Health Care Home by integrating care for physical health conditions.

This is a Full-time, benefit eligible position working Monday - Friday. This Candidate will cover Cabool or West Plains as well as Howell, Texas and neighboring counties.
