Job descriptionLovelace Women's Hospital is a 53-bed advanced neonatal intensive care unit, family care unit, an award-winning breast care center and the first robotic surgery program in the state recognized as a Center of Excellence in Robotic Surgery by Surgical Review Corporation. The Clinical Case Manager assesses the social, psychosocial, cultural, environmental and financial situations concerning the patient and family. He / she is a patient / family advocate, promoting rights and dignity and striving to involve the patient / family in aspects of care as appropriate. The Clinical Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care. BS or BA in Social Work or Associates Degree in Nursing or Diploma of Nursing Social Worker : Medical Social Worker. Must have current license as Social Worker from New Mexico Board of Social Work Examiners, and basic life support certification. Registered Nurse : Valid NM Nursing license or NM compact license (within 90 days of hire) and BLS; HIRING DEPARTMENT MAY ALSO REQUIRE ACLS and or PALS. Master's Degree in Social Work or Bachelor's Degree in Nursing Two years of medical social worker or RN Case Manager experience Social Worker : Licensed Clinical Social Worker Registered Nurse : CCM or ACM
