Hospice Chaplain PRN

The Hospice Chaplain plays a vital role in providing spiritual and emotional support to patients facing end-of-life challenges and their families. This position involves offering compassionate care that respects diverse religious beliefs and cultural backgrounds, helping individuals find peace and meaning during difficult times. The chaplain collaborates closely with interdisciplinary hospice teams to ensure holistic care that addresses physical, emotional, and spiritual needs. They also facilitate grief counseling and bereavement support for families and staff, fostering resilience and healing. Ultimately, the Hospice Chaplain contributes to enhancing the quality of life for patients and their loved ones by providing comfort, guidance, and hope throughout the hospice journey.

As a Hopestone Hospice Chaplain, you will:

• Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
• Provide support for the spiritual needs of the patient/family.
• Attend interdisciplinary team meetings and participate in the patient care planning process.
• Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements of the agency.
• Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
• Promote Agency philosophy and administrative policies to ensure quality of care.
• Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.

Requirements

• One year of experience as a Chaplain in health care preferred
• One or more years of Hospice experience required
• CPR Certification
• Valid Driver's License
• Valid Auto Insurance
• HomeCare HomeBase experience preferred
