Location : Grand Junction, CO
Schedule : Flexible Part-Time / PRN (Weekday Hours)

We're seeking a Part-Time or PRN Speech Therapist to provide compassionate, individualized therapy for patients in the comfort of their homes.

What You'll Do
As part of our interdisciplinary home health team, you will :
Evaluate and treat patients with speech, language, voice, cognitive, and swallowing disorders.
Develop and implement personalized treatment plans to meet each patient's goals.
Collaborate closely with RNs, PTs, OTs, and caregivers to ensure coordinated, holistic care.
Educate patients and families on techniques to improve communication and daily function.
Document all care provided accurately in our electronic health record (HCHB training provided).
Travel locally within Mesa County for home visits (mileage and drive time reimbursed).

What You'll Need
Current Colorado Speech-Language Pathology (SLP) license .
Certificate of Clinical Competence in Speech-Language Pathology ( CCC-SLP ) preferred.
Experience in home health, rehab, or medical settings preferred (new grads welcome).
Strong communication and documentation skills.
Reliable transportation, valid driver's license, and auto insurance.
