Licensed Practical Nurse (LPN) – Home Care (Flexible Hours)

About the Role

We are seeking a caring Licensed Practical Nurse (LPN) to complete supervisory visits and provide limited hands-on care in client homes. We will train motivated LPNs who are interested in learning home care.

What Youll Do

Conduct supervisory visits to monitor and support HHA staff

Perform client assessments within scope

Document all visits in AxisCare

Provide occasional nursing care such as vitals medication reminders and simple treatments

Report changes in condition to the RN and leadership team

Complete minimal weekend visits if needed (not full shifts)

Schedule

Flexible self-paced scheduling

Preferred weekday availability starting at 7 : 00 AM

Full-time or part-time options available

Requirements

Active LPN license in Massachusetts

Valid drivers license and reliable transportation

Good communication and time-management skills

Comfortable traveling to client homes
