POSITION SUMMARY
The Care Coordinator works directly with the Triage Team to review and process incoming referrals to ensure timely and appropriate care for referred patients.
PRIMARY RESPONSIBILITIES
1. Works with the Triage team to facilitate patient access to EMG’s services.
2. Ensures complete registration information is received and entered into Nextech prior to the appointment including optometrist, PCP, specialist(s), insurance information and insurance referral information when required.
3. Documents the “notes” section of the patient account regarding any pertinent information.
4. Documents all patient interactions within the patient’s medical record.
5. Schedules new patient appointments to the mutual satisfaction of the patient and provider.
6. Provides patient education regarding what to expect as part of their care at EMG, including preparation and mailing of new patient paperwork.
7. Communicates to patients in a manner that instills understanding and confidence about their upcoming appointment.
8. Communicates with referring Optometrists with their patient’s appointment information.
9. Provides a high level of customer service by demonstrating compassion and caring in dealing with others.
10. Scans paper medical records into the patient’s electronic medical record.
11. Monitors and manage medical information shared electronically (EMR, virtual faxes) in a timely manner.
12. Participate in duties interdepartmentally as deemed necessary, by a member of management and / or doctor(s).
13. Demonstrates flexibility to meet changing demands on daily basis while also accomplishing tasks.
14. Demonstrates the ability to re-prioritize workloads and assignments for the team on short notice.
15. Attends meetings, as assigned, and participates in educational activities to keep skills up to date.
16. Maintains open communication with PSRs and Surgical Planning teams to maximize provider schedules.
17. Performs other duties necessary to maintain the overall efficiency and continuity of EMG operations.
18. Monitors operating standards and report issues to leadership in a timely manner.

WORK ENVIRONMENT
Work is completed in an office and / or clinical healthcare setting with little exposure to harsh conditions and hazards. Training, Certification and / or Personal Protective Equipment is provided when necessary. Incumbent will have the ability to move freely and may be required to sit or stand for long periods of time. May be required to balance, bend / stoop, stretch, push / pull, kneel, reach and lift up to 25 lbs. to perform essential duties and responsibilities of the role. Hand and eye coordination, finger dexterity and the ability to communicate both verbally and in writing. Is required to travel to and from office locations as needed.
Daily operation of various office equipment : fax machine, telephone & voice mail operating system. Stationary / laptop computer, e-mail system, & copier.
