Registered Nurse (RN) – Home Health – Lower Sussex County, DE

Registered Nurse (RN) Responsibilities :

Perform home health visits with adult patients throughout lower Sussex County

Follow a personalized plan of care based on physician orders

Administer skilled nursing care including wound care, medication administration, and treatments

Monitor patient status and communicate changes to the clinical team

Coordinate care with other disciplines (PT, OT, HHAs, etc.)

Accurately document patient care using an electronic medical record (EMR) system

Qualifications :

Current RN license in the state of Delaware

Strong communication and time management skills

Ability to work independently and travel to patient homes within the assigned territory
