• Other details : The facility is looking for a provider to cover on-site clinical hours only. There are an average of 18 patients per shift. The position includes some chronic pain management with chart closures required within 72 hours. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with the possibility of 10-hour shifts, and providers must see a minimum of 18 patients per day, including both pediatric and adult cases, though Family Medicine providers will also be considered. Both MD and DO candidates are accepted, and board-eligible physicians will be considered if within five years of residency completion. An active DEA is required prior to the start date. Credentialing typically takes days, averaging six weeks if all required documents are submitted promptly. Immunizations required before the start date include the flu vaccine, TB test or X-ray (within 90 days of the start date), positive MMR, Hepatitis B, and Varicella Zoster titers or completed immunization series, and a Tdap vaccine administered within the last 10 years. A hiring decision is generally made within 7 14 days.
