As a key member of Intermountain Home Services, you’ll work directly within healthcare facilities to support seamless patient transitions to home-based care. This position is ideal for a compassionate, detail-oriented RN who thrives in a collaborative environment and is passionate about patient-centered care.

What You’ll Do

As an RN Liaison, you’ll be the vital link between hospital care and home-based healing. Your role will include :

• Serve as a clinical ambassador for Intermountain Home Services, educating referral sources and promoting appropriate home-based care options
• Assess patients for eligibility and readiness for home services, coordinating timely transitions from hospital to home
• Collaborate with physicians, case managers, and interdisciplinary teams to develop and implement patient-centered care plans
• Support discharge planning by reviewing medical records, identifying care needs, and ensuring accurate intake documentation
• Communicate and document all care coordination activities clearly and professionally
• Address barriers to care—financial, logistical, or psychosocial—while advocating for patients and families across the care continuum
• Ensure timely and accurate completion of orders and documentation to support seamless care transitions
