Remote Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse - LPN - LVN - Intake Coordinator for a remote day shift position.

Key Responsibilities

Conduct preliminary health assessments for newly enrolled patients and resolve their questions

Identify and coordinate community resources and provide patient education on chronic condition management

Assist with medication management and ensure timely delivery of services to patients

Required Qualifications, Training, and Education

Active Compact / Multi-State license as an LPN or LVN

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a fast-paced environment

Must have a home office or HIPAA-compliant workspace with high-speed Wi-Fi
