Job descriptionProvides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting. Possesses a clear understanding of discharge and transition planning and linkage with community resources. Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations. Provides support as active member of the treatment team. Will perform other duties as assigned by supervisor. BLS (Basic Life Support) Behavioral Health - 1 year Child and Adolescent - 2 years (SIPP Only) Acute Care - Preferred 1 year Bachelors - Counseling / Health Education / Related Field / Social Work / Human Services / Psychology / Nursing Master's in human services - Preferred Health, Dental, Vision Paid time off Tuition reimbursement 401k match and additional yearly contribution Yearly performance appraisals and team award bonus Community discounts and more AND the Chance to be part of an amazing team and a great place to work!

BayCare Health Systems is currently seeking a Discharge Planner 1 to join our outstanding and compassionate team at the Behavioral Health Center!

- Shift Hours : 8 : 00 am- 4 : 30 pm
- Weekend Work : Occasional
