Licensed Practical Nurse (LPN) – Home Health (Full-Time)Location: Salem, Oregon

Position Summary:
The Licensed Practical Nurse (LPN) provides skilled nursing care under the supervision of a Registered Nurse (RN) and in accordance with the patient’s individualized plan of care. The LPN collaborates with the interdisciplinary team to ensure patient-centered care that promotes health, healing, and safety in the home environment.
Key Responsibilities:
Provide direct patient care in the home setting according to the plan of care established by the RN and physician.
Administer medications and treatments as prescribed, documenting care accurately and timely in the EMR.
Monitor and report changes in patient condition to the supervising RN and physician promptly.
Perform wound care, catheter care, injections, and other nursing procedures as ordered.
Educate patients and caregivers on disease management, medication administration, and home safety.
Collaborate with other disciplines including physical therapy, occupational therapy, speech therapy, and social work to ensure coordinated care.
Participate in case conferences, staff meetings, and in-service training.
Maintain compliance with all federal, state, and company policies including infection control, HIPAA, and patient rights.
Ensure timely and accurate documentation to support billing, compliance, and continuity of care.
Qualifications:
Current Oregon Licensed Practical Nurse (LPN) license in good standing.
Minimum of one year of clinical experience, preferably in home health, medical-surgical, or geriatric care.
Current CPR/BLS certification.
Valid driver’s license, auto insurance, and reliable transportation.
Strong organizational, clinical, and communication skills.
Comfort with technology and electronic documentation.
Schedule & Compensation Full-time position with flexible scheduling based on patient needs.
