Job Summary :

We are seeking a dedicated and experienced Registered Nurse (RN) - Case Management to join Adventist - St. Helena on a travel assignment. The RN will play a key role in collaborating with the healthcare team to provide exceptional patient care through case management. This position requires expertise in acute hospital medical case management, discharge planning, utilization review, and status management.

Responsibilities :

Collaborate with patients, families, nurses, physicians, and multidisciplinary teams to develop, implement, and revise case management plans.

Serve as a consultant and liaison to optimize patient outcomes throughout hospitalization.

Coordinate the continuum of care from pre-admission through post-discharge for a defined patient population.

Assist with discharge planning and ensure efficient patient flow through hospital stay.

Utilize Cerner / PowerChart for patient charting and documentation.

Requirements :

Current Registered Nurse (RN) licensure in the state of practice (California).

Must have Cerner / PowerChart experience .

Recent (within last year) acute hospital medical case management experience in a hospital setting.

Strong knowledge of discharge planning, utilization review, and patient status management.

Ability to work day shifts from 8 : 00 AM to 4 : 30 PM, 40 hours per week.

Preferred Qualifications :

Bachelor’s Degree in Nursing (BSN).

Case management certification.

Prior experience in a care management role.
