Registered Nurse (RN) - MLTC Nurse Care Manager

Location : Manhattan & Brooklyn, NY

Employment Type : Full-Time

About the Opportunity :

We are seeking passionate and dedicated Nurse Care Managers to join a growing Managed Long-Term Care (MLTC) team. In this role, you will help older adults and individuals with chronic health needs maintain their independence and quality of life in their homes and communities.

Open Positions :

MLTC Nurse Care Manager - Manhattan (Bilingual Spanish, Mandarin, or Cantonese)

MLTC Nurse Care Manager - Brooklyn (Bilingual Cantonese, Mandarin, Spanish, or Russian)

Key Responsibilities :

Collaborate with members and their families to support the most independent living situation possible.

Conduct comprehensive in-home assessments and develop individualized care plans.

Monitor members' health status and coordinate care across multiple providers.

Authorize and manage covered services to ensure quality and cost-effective care.

Partner with members, families, caregivers, primary care practitioners, and the interdisciplinary team to ensure continuity of care.

Qualifications :

• Bachelor of Science in Nursing (BSN)
• Experience at a health plan (emblem, VNS, etc.)
• Bilingual in Mandarin, Cantonese, Spanish (Spanish required for Bronx), or Russian preferred
• OR Extensive home care experience

Compensation & Benefits :

Annual Salary : $105,000 - $115,000

Sign-on Bonus : $20,000 total ($5,000 paid every six months, less applicable taxes)
