Contract Length : 6 months (potential for extension)

Schedule : Monday Friday, 8 : 00am 5 : 00pm (40 hours / week)

Position Overview

We are seeking a dedicated Nurse Practitioner to join a community health organization in San Jose, CA. The provider will work in a school-based setting serving both students and members of the local community. This is a full-time, Monday through Friday role focused on providing primary care services to patients of all ages.

Responsibilities

Provide comprehensive primary care to patients across the lifespan.

Support chronic disease management and assist in meeting HEDIS quality measures.

Collaborate with multidisciplinary staff to ensure high-quality patient care.

Maintain accurate and timely charting using Epic EMR.

Participate in a full week of mandatory orientation prior to starting.

Patient Volume : 21 patients per day (21+ preferred)

Patient Population : All ages (pediatric through adult)

Setting : Outpatient / school-based primary care

Support Staff : On-site clinical and administrative support

Orientation : One full week (Monday Friday, required attendance)

Credentialing Timeframe : 30 days or less

Requirements

Active California NP license (required)

Board Certified Nurse Practitioner

BLS certification (required)

Experience with Epic EMR preferred

Must be comfortable working with children and adults

Willingness to commit to the full 6-month contract

BACH s needs may vary across sites, including school-based, brick-and-mortar, and mobile clinics. Candidates available for multiple locations will be prioritized.
