2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment.

Experience with HMOs, IPAs, etc.

Job Summary

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring effective communication among healthcare providers, and facilitating discharge planning. This position requires a strong background in nursing with experience in various clinical settings, including acute care, outpatient, and specialty areas.

Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
