Job Title
Acts as a team member in the delivery of quality rehabilitation services under the supervision of a therapist.
Essential Functions
- Assists the therapist in the provision of rehabilitation services under the direction of a therapist, consistent with department policies and procedures.
- Informs patients and significant others about treatments being received, equipment being used, and other related activities.
- Performs clerical duties such as answering telephones, scheduling patients, patient registration, processing physician orders, computer data entry / retrieval, completing forms, documenting communication / care, and preparing patient charts.
- Participates in selected department quality monitoring activities.
- Schedules patients for revisits.
- Cleans equipment / work area / patients environment.
- Attends appropriate meetings and conferences.
- Provides transportation of the patient / client using the employees personal vehicle.
- Maintains inventory of materials, forms, and supplies according to departmental standard.

Required : High School Diploma or equivalentPreferred : 6 months of relevant experienceRequired : Successful completion of competency based and on the job training orientation programRequired : CRT-Basic Life Support (BLS) - AHA American Heart Association or CRT-Basic Life Support (BLS) - ARC American Red CrossRequired : LIC-Drivers License - STATE_MI State of MichiganPrimary Location : SITE - Rehab & Nursing Leffingwell - 1001 Leffingwell - Grand Rapids
