Job description

Hospital Pharmacist - Travel Position

Location : Wichita Falls, TX

Job Type : Travel Contract

Duration : 13 weeks

Shift : 5x8s, Weekdays with every third weekend required

Pay : $2681 / week

Required Certifications & Competencies

  • ACLS (Advanced Cardiac Life Support) - AHA Instructor, Provider, or RQI acceptable
  • BLS (Basic Life Support) - AHA Instructor, Provider, or RQI acceptable
  • PALS (Pediatric Advanced Life Support) - AHA Instructor, Provider, or RQI acceptable
  • NRP (Neonatal Resuscitation Procedure) - Required as applicable
  • CHEMO (Chemotherapy) - Required as applicable
  • TNCC (Trauma Nursing Core Course) - Required as applicable
  • EFM (Electronic Fetal Monitoring) - Required as applicable
  • ENPC - Required as applicable
  • NIH Stroke Certification - Required as applicable
  • Medication Test / Pharmacology Competency Exam - Must meet or exceed passing score
  • Competency Exam - Must meet or exceed passing score
  • Tech Cert Verification (National) - Required within 30 days of start
  • Orientation Safety Manual Acknowledgement - Required, update annually
  • State License Verification - Required within 30 days of start
  • NURSYS Verification (RN, LPN / LVN, AP) - Required within 30 days of start

Required Documents & Compliance

  • I9 or Attestation
  • Government Issued Photo ID (Driver's License / State ID)
  • E-Verify or Attestation
  • Background Check - Within 1 year of start, renewed annually
  • EPLS / GSA / SAM - Required within 30 days of start
  • HHS / OIG - Required within 30 days of start
  • 2 References within 3 years of start
  • Employment Application / Resume
  • Skills Checklist - Required, update annually

    • Health & Immunization Requirements

  • Physical Exam - Within 1 year of start, must be completed within 72 hours of contract signing
  • Tetanus / Diphtheria / Tdap - Vaccine within 10 years of start
  • Negative TB Test or Chest X-Ray - Within 1 year of start; chest X-ray within 5 years if positive PPD
  • TB Questionnaire - Required for all candidates
  • Rubeola (Measles) - 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
  • Mumps - 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
  • Rubella - 2 vaccines or positive titer required; exemptions apply for candidates born before 1957
  • Hepatitis B - Positive titer, series, or declination required
  • Varicella - 2 vaccines, positive titer, or history of chickenpox required
  • Flu Seasonal Vaccination - Required Sept-May; medical / religious exemptions subject to facility review
  • COVID-19 Vaccine

    • Facility Specific Requirements

  • Flu Medical Exemption (if applicable)
  • Flu Religious Exemption (if applicable)
  • Parking Policy Acknowledgment - Must be completed prior to start
  • Drug Testing - Negative 9 Panel Drug Screen within 1 year of start; plan for completion within 72 hours of contract signing
