Job Summary :  This role has two essential contributions to the overall success of the patient care journey which include 1) participating as a team member in the Admissions department performing telephonic intake calls with patients using social work assessment skills to identify patients in need of social work services to aid in addressing Social Determinates of Health and in meeting clinical outcome goals; 2) responsible for the provision, direction, supervision, and evaluation of medical social services to clients in the home environment, in accordance with agency policies and procedures, applicable health standards, governmental laws and regulations. Proactively meet the needs of and assist the client in the achievement of individualized and measurable outcomes.
