Program : Adult Assertive Community Treatment (ACT)

Location : White Plains, NY / Mobile / Hybrid

Hours Per Week : 35

FLSA Status : Full Time, Exempt

Salary Range : $58,350-$86,600 annually

Summary Description : We have an exciting opportunity for a full-time Clinician in our Assertive Community Treatment (ACT) program. The ACT team provides non-traditional community-based mental health treatment to promote the recovery and community integration of 68 adults with serious mental illnesses and co-occurring substance use disorders. The specialist delivers routine treatment services, integrates family goals and services, and provides family psycho-education. The Family Specialist supports consumers to achieve wellness, recovery, and positive relationships in the community, utilizing evidence-based practices and a trauma-informed and person-centered approach.

Key Responsibilities :

  • Conduct comprehensive assessments of psychiatric history, mental status, diagnosis, physical and dental health, substance use, education, employment, social development, functioning, daily living activities, and family relationships.
  • Address housing, income support, education, vocational training, social supports, employment, and primary care needs.
  • Involve families and social supports in the consumer’s treatment with the team.
  • Provide engagement, problem-solving, wellness self-management, medication support, family support and treatment, individual mental health, relapse prevention, harm reduction, and substance use counseling.
  • Provide 24-hour crisis intervention on a rotating basis.
  • Deliver 80 percent of treatment contacts in the community.
  • Document client progress according to established methods and procedures.
  • Participate in daily morning team meetings and treatment planning review meetings.
  • Follow ACT policies & procedures, agency policies, and maintain professional standards.
  • Use client data for clinical decision-making, quality improvement, and tracking progress.
  • Commitment to cultural competency, diversity, and inclusion.
  • Share client data with clients to facilitate shared decision-making.
  • Other duties as assigned

Key Competencies :

  • Experience understanding family systems based on diversity of cultural and ethnic backgrounds
  • Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
  • Extensive knowledge of Assertive Community Treatment, family community support services such as NAMI, and community resources
  • Use of Motivational interviewing, recovery and trauma-informed approaches when delivering services
  • Proficient in computers
  • Highly organized and self-motivated
  • Familiarity with and ability to use an Electronic Health Record system.
  • Demonstrated commitment to diversity, equity, inclusion and cultural humility.
  • Key Performance Indicators (KPIs) :

  • Positive feedback from youth participants on the effectiveness of mental health services.
  • Successful achievement of treatment goals and positive outcomes for participants.
  • Compliance with program guidance and ethical / legal standards in client record-keeping.
  • Active participation in multidisciplinary teams and case conferences.
  • Continued professional development and training in youth mental health.
  • Key Competencies :

  • Requirements :

  • Active LMHC, LMSW, or LCSW with a Master’s degree in the Human Services field; limited permit holders considered.
  • Experience using culturally competent treatment approaches.
  • Must be legally eligible to work in the United States.
  • Basic computer knowledge.
  • Reliability, ability to work independently, and maintain confidentiality.
  • Must maintain a valid Driver’s License and a satisfactory driving record.
  • Must maintain pre-employment clearances by applicable regulatory clearances as required.
  • Special Considerations :

  • This is a mobile, community-based position
  • Hours per week, reporting structure, and location are subject to change per program needs.
  • Mandated Reporter :

    This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter. You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
