Job description
  • The Medical Director will be responsible for leading, as adyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.
  • The At Home division is an interdisciplinary team that focuses on primary care and transitional care support for our highest-need patients in their homes.
  • The primary responsibility for the Medical Director is direct management, oversight, and coaching of the local team, and is part of the At Home divisions leadership team which helps develop programs (i.e., complex care coordination, chronic disease management, psychosocial support, etc.) throughout the division.
  • The Medical Director will see patients in their homes up to 3days aweek alongside their provider colleagues. Additionally, we see this leader being akey thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program.
  • Were looking for motivated, experienced Primary Care Providers
  • Board Certified IM or FM or Geriatric Medicine
  • Experience with managed care and / ?or value-based practice
  • A commitment to older adults and to low-income patients
