Bachelor of Science in Nursing (BSN) degree, active Nurse Practitioner (NP) license and minimum 1+ years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

There is an open Per Diem Nurse Practitioner (NP) Home Health Flexible Schedule job in Fort Morgan,CO. The Per Diem Nurse Practitioner (NP) will conduct home health risk assessments within Morgan County, CO. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Fort Morgan, COand the surrounding Morgan County, CO.Morgan County, COincludes locations like Brush, Hillrose, Log Lane Village, and Wiggins.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities :

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
• Provide education on medical conditions and medications
