LPN - Nephrology Care Associate

Overview : Join a compassionate healthcare team as a Licensed Practical Nurse specializing in nephrology, working full-time night shifts. This role involves providing direct nursing care to assigned patients under the guidance of a Registered Nurse, ensuring their safety and well-being in a supportive environment.

Key Responsibilities :

• Participate in patient assessments and collaborate with interdisciplinary teams to develop and follow individualized care plans.
• Deliver nursing care tailored to the age-specific physiological needs of patients, as directed by supervising nurses or authorized personnel.
• Maintain accurate and detailed patient care records and administer medications as prescribed by physicians.
• Ensure a safe environment by recognizing and responding appropriately to life-threatening situations.
• Engage in performance improvement initiatives and monitor quality of care standards.
• Foster collaborative relationships with healthcare staff and support patients' cultural and spiritual practices within medical guidelines.
• Contribute to cost containment efforts and accurate patient service documentation.
• Perform additional duties as assigned to support patient care and hospital operations.

Education : High school diploma or equivalent required.

Licensure & Certifications : Current Licensed Practical Nurse licensure in a Compact state or the specific state of practice; Basic Life Support (BLS) certification from the American Heart Association.

Experience : No prior experience required; positive interpersonal skills and effective verbal and written communication are essential.

Physical Requirements : Ability to communicate effectively, observe patients closely, and perform physical tasks such as standing, walking, bending, and lifting for extended periods. May involve exposure to blood or other potentially infectious materials.
