North Platte Valley Medical Center is seeking dedicated and skilled LPNs, LVNs to join our Extended Care / LTC team. As part of a collaborative, team-oriented unit, you will work alongside RNs, CNAs, LPNs, and interdisciplinary staff to deliver high-quality, patient-centered care in a long-term care environment.
Position Overview
This role provides direct nursing care, observes and contributes to care plans, implements interventions, and documents patient care in accordance with department policies, standards of care, and state scope of practice. You will have the opportunity to critically think, work autonomously, and demonstrate leadership and delegation skills while supporting patients and families.
Key Responsibilities
Provide direct, evidence-based care to patients as directed by the provider or supervising RN.
Assist with procedures, monitoring, and lab or diagnostic tests within your scope of practice.
Document all care accurately and in a timely manner in the patient medical record.
Serve as a resource to staff to support quality patient care delivery.
Must have a teamwork mentality and willing to work alongside CNA's on the floor in a long-term care based setting
Ability to respectfully and appropriately communicate with residents and their family members regarding patient care
Must maintain HIPAA compliant and follow all policies regarding PHI.
Collaborate with interdisciplinary team members to ensure comprehensive care.
Integrate standards of care into patient care plans based on diagnosis and age-specific needs.
Perform other duties as assigned, demonstrating flexibility and professionalism.
Must maintain professionalism during all hours of work
Must attend all mandatory departmental or hospital wide meetings.
Requirements
Licensure :
Wyoming LPN / LVN license with IV Certification (LPN IV) required for LPN / LVN roles.
Certifications : BLS required.
Background check, drug screening, and TB test required upon hire.
Experience in a rural hospital setting preferred but not required.
Ability to work in a fast-paced, high-stress environment and have the ability to prioritize workload.
Strong communication, critical thinking, and leadership skills.
Work Schedule
Standard 12-hour shifts. NPVMC promotes a 12-hour, 7 on 7 off rotating schedule.
