Become the Case Manager You've Always Aimed to Be
Work collaboratively with an interdisciplinary team to guide treatment plans based on patient needs.Establish tentative discharge plans and contingency measures in coordination with the care team.Participate in planning and executing the patient discharge experience.Monitor patient experiences and ensure quality and timeliness of services.Facilitate weekly team conferences to review treatment plans and modifications.Complete necessary documentation and case management addendums.Stay updated on relevant regulations and departmental policies.Analyze case management reports and develop action plans for improvement.Participate in Acute Care Transfer meetings to address trends and outcomes.Engage with patients and families during initial visits within 24 hours of admission.Conduct assessments and complete documentation within 48 hours of admission.Educate patients and families about rehabilitation and the Case Manager's role.Coordinate family conferences as needed.Assist patients in securing necessary resources for timely discharges.Ensure compliance with regulations regarding orthotics and prosthetics.Make timely referrals to post-discharge providers and document accordingly.Verify discharge and payor-related information is accurate in patient records.Contribute to utilization reviews by collecting data and participating in trend analysis.Participate in the case management on-call schedule as needed.Qualifications Required
License or Certification :Must be qualified to independently complete assessments in accordance with your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).Active licensure is required where applicable.Eligibility for CCM® or ACM certification upon entering this position or within two years of hire.CCM® or ACM certification is mandatory or must be obtained within two years if assuming the Case Manager II role.Minimum Qualifications :
For nursing positions, an Associate Degree in Nursing is required; RN licensure is mandatory, with a BSN preferred.For other licensed or certified health care professionals, a minimum of a bachelor's degree is necessary, with a graduate degree preferred.Two years of rehabilitation experience preferred.The Encompass Health Commitment
