• Patient Care and Assessment : Conducts thorough assessments of patients' health status, including vital signs, medical history, and current symptoms. Develops and implements individualized care plans based on these assessments.\n
• Nurse Triage : Performs initial triage to prioritize patient care by evaluating the urgency of their conditions. Determines appropriate next steps for treatment or referral, ensuring timely and effective care.\n
• Care Coordination and Communication : Collaborates with physicians, specialists, and other healthcare providers to coordinate care, ensure effective communication, and manage patient treatments and follow-ups.\n
• Patient Education and Support : Provides education to patients and their families about managing health conditions, medication administration, and lifestyle changes. Offers emotional support and answers questions to help patients navigate their care.\n
• Administrative and Documentation Duties : Maintains accurate and up-to-date patient records, including charting care provided, documenting patient interactions, and managing appointment scheduling and other administrative tasks.\n\nWhat you will need : \n
• A Diploma from an accredited school / college of nursing is required - BSN or ADN\n
• BLS provider (accredited by the American Heart Association) required prior to hire\n
• Current Registered Nurse License State of Illinois\n
