Job Description

Certified Pharmacy Technician (CPhT) certification and 1+ years Pharmacy Technician experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for a skilled Clinical Pharmacist with expertise in infusion and home infusion pharmacy services. This role is key in ensuring the safety and effectiveness of pharmacy operations across multiple infusion sites and pharmacies that supply these locations.

Responsibilities :

Review and verify prescriptions for accuracy and safety.

Evaluate patient records to determine appropriate medication therapies.

Provide counseling to patients on medications, ensuring understanding and adherence.

Identify therapeutic inpatibilities and manage dispensing processes.

Monitor patient drug therapies and remend interventions as necessary.

Oversee and validate pharmacy technician activities and documentation where applicable.

Collaborate closely with teams in Reimbursement, Quality Management, Nursing, Clinical Services, and Pharmacy Services.

Qualifications / Desired Experience :

Current and active California pharmacist license in good standing.

Proven experience in infusion or home infusion pharmacy settings.

Familiarity with the CPR+ system is highly preferred.

Ability to manage multiple sites and coordinate with various departments effectively.

Strong clinical judgment and patient care management skills.

Additional Details :

This direct hire role may consider contract-to-hire arrangements due to the mitment involved with relocation to or within the Bay Area.
