About The Position

Description :

This role involves providing hospice care, assisting patients and their families during difficult times.

Shifts will be Monday to Thursday from 2 PM to midnight.

Call responsibilities will occur from midnight to 8 AM on the same days.

Call will be shared between two RNs.

Nurses may receive triage calls or visit patients as needed during on-call hours.

The position requires a strong background in hospice care and end-of-life support.

Candidates must demonstrate skills in pain management, patient assessment, and family education.

Additional expected competencies include advanced directives and medication management.

Experience with charting systems is preferred.

Interdisciplinary support from social services and pharmacy is integral to patient care.

The role primarily serves adult patients, with occasional pediatric care.

Providers will be responsible for managing a variety of patient diagnoses in a home hospice setting.

Proper documentation of hospice eligibility must be maintained.

An understanding of both pharmacologic and non-pharmacologic pain interventions is essential.
