Job Title :   Home Health Nurse (RN / LVN)

Location : Oxnard, CA / Ventura County / Thousand Oaks Area

Job Type : Per Diem / Full-Time / Part-Time (Flexible Scheduling Available)

Start Date : ASAP

Pay : Competitive rates (based on experience and license type)

∙RN : $75–$115 per visit

∙LVN : $35–$40 / hour or $50–$68 per visit

What You'll Do :

🏠 Deliver skilled nursing care to patients in their homes in accordance with physician

orders and individualized care plans.

📋  Conduct patient assessments and ongoing evaluations .

💊  Administer medications, wound care, injections, and other treatments as prescribed .

📞 Collaborate with interdisciplinary team members, including physicians, therapists, and

case managers.

📝 Educate patients and families on disease management, medication administration, and

health maintenance.

👨‍👩‍👧‍👦 Complete and submit clinical documentation accurately and timely.

What We're Looking For :

✅ Active California RN or LVN license

✅ Minimum of 1 year of nursing experience (Home Health experience preferred but not

required)

✅  Current BLS certification

✅ Valid driver’s license, reliable transportation, and proof of auto insurance

✅ Strong clinical assessment and communication skills

✅ EMR documentation experience a plus

✅  DSL portal experience preferred
