Responsibilities :

  • Facilitate group and individual therapy sessions for patients struggling with addiction.
  • Conduct intakes and assessments to gather patient information and guide treatment plans.
  • Collaborate with the treatment team to develop and implement individualized care plans.
  • Maintain accurate and compliant patient records in accordance with 245G and 1115 Demonstration Waiver requirements.
  • Participate in clinical consultations and staff meetings to ensure continuity of care.

    • Requirements :

  • Current and active Licensed Alcohol and Drug Counselor (LADC) credential in Minnesota.
  • Experience providing substance use disorder counseling and case management
  • Strong documentation skills and experience using electronic health record (EHR) systems.
  • Ability to respond to on-call requests within a specified timeframe.
  • Excellent communication, crisis intervention, and problem-solving skills.

    • Compensation : $70-75K / Year with a $7,5000 Sign on bonus

    Actual compensation may be determined by various factors such as licensure, experience, skillset, internal equity, schedule and / or location. Estimates are factored on a full time FTE.
