About the Role We're looking for a mission-driven Licensed Practical Nurse (LPN) to support members across Central Florida through telehealth and in-person care. This is a hybrid role — you'll begin by working onsite at least three days per week (in partner communities and clinics) and will gradually transition to a more remote telehealth-based schedule as your patient panel grows. You'll be a trusted guide for our members — helping monitor their health, close care gaps, and act before small issues become big ones. In this role, you will :Manage an assigned panel of members (mostly Medicare / older adults)Conduct scheduled telehealth and in-person check-ins to assess needs, coach on medications and lifestyle habits, and identify early warning signsRecord findings, update care plans, and escalate clinical concerns to an RN or NP as neededCoordinate follow-up visits, lab work, and community resourcesCollaborate with the Preemptive Health team to deliver measurable outcomes — better control, fewer hospital visits, and happier members ? ⚕️ Who You AreActive Florida LPN license (or Compact)2+ years of experience in primary care, home health, telehealth, or care managementCompassionate communicator with strong follow-throughTech-comfortable (Google Meet, Docs, EHR platforms) ? — laptop required for work and interviewsReliable, organized, and motivated to grow within a mission-driven team ?
