Travel Physical Therapist (PT) - $2,692 per week in Saint Louis, MO

Job Title : Physical Therapist - Home Health Profession : Therapy / Rehabilitation Specialty : PT Home Health Duration : 13 weeks Shift : Day Hours per Shift : 8 hours Experience : Minimum of 1 year of experience required License : Valid state license required at the time of submission Certifications : BLS required Must-Have : Home Health experience Description : No requests for time off from 12 / 22 to 1 / 5. The average patient visit ratio is 5-20 visits per day. First-time travelers are accepted. Weekend work required every 6th weekend. No on-call work is required. Clinician will not float outside their scope to meet facility needs. Scheduling is Monday through Friday with every 6th weekend required. Local candidates are accepted with specific radius restrictions. Pending licenses are accepted for submission. Territory mileage radius is 40-80 miles. Mileage will be reimbursed. No more than 7 days of requests for time off are allowed for the contract period. Candidates should have experience in Home Health settings. The facility uses Epic for charting. References may be required. Specific charting experience is required. Background checks will be conducted prior to hiring. Compliance with specified deadlines is mandatory. All candidates must have a valid Social Security Number and date of birth upon submission. Laboratory tests should be completed within 48-72 hours after signing. Candidates should indicate their interest in extending beyond 13 weeks if applicable.
