Rosenberg Health & Rehabilitation Center, proudly managed by Brightstone Healthcare Services , is seeking a dedicated and compassionate PRN Occupational Therapist (OT) to join our in-house rehabilitation team in Rosenberg, Texas.

Position Summary

As an Occupational Therapist, you will assess, plan, and implement therapy interventions to help residents achieve their highest level of independence in daily living activities. You’ll play a key role in improving outcomes and promoting wellness within our skilled nursing community.

Key Responsibilities

• Evaluate residents to identify functional limitations and rehabilitation potential.
• Develop and implement individualized treatment plans to promote independence.
• Provide skilled OT services based on physician orders and established care plans.
• Document evaluations, treatments, and progress reports accurately and timely.
• Educate residents, caregivers, and staff on adaptive techniques and equipment use.
• Collaborate with the care team to ensure comprehensive, resident-centered care.
• Maintain compliance with company policies, professional standards, and regulatory requirements.
