Position Overview

As a Physical Therapist, youll provide evidence-based therapeutic care across multiple settings including outpatient rehab, skilled nursing, and assisted living facilities . Youll be empowered to design customized treatment plans, collaborate with interdisciplinary teams, and make a meaningful impact on the lives of adult and geriatric patients. Choose a permanent placement or take advantage of our traveler-friendly assignment option.

Key Responsibilities

Develop and implement individualized treatment plans based on physician referrals

Conduct patient evaluations and deliver therapeutic interventions

Track progress through detailed documentation and regular assessments

Collaborate with nurses, therapists, and support staff to optimize outcomes

Participate in care planning and team meetings

Provide education and support to patients, families, and staff

Offer leadership support in the Directors absence

Qualifications

Graduate of an accredited Physical Therapy program (DPT or equivalent)

Active New York State PT license (or eligibility)

Strong interest in adult, subacute, and geriatric care

Excellent communication and team collaboration skills

Leadership ability and willingness to mentor rehab aides or junior staff

Experience preferred, but new graduates are encouraged to apply!
