Job Title : Hospital Social Worker (LCSW) – Acute Care Evening Shift
Location : Lawrenceville, GA
Schedule : Monday to Friday | 3 : 30 PM – 12 : 00 AM
Duration : 8 weeks
Rate : $2200 weekly
Position Overview :
We are seeking a dedicated Hospital Social Worker (LCSW) to support patients and families in a high-acuity hospital setting. The ideal candidate will provide emotional, psychological, and resource-driven support during the evening shift.
Key Responsibilities :
- Deliver crisis counseling and psychosocial evaluations
- Collaborate with healthcare teams for patient-centered care plans
- Assist in discharge planning and referrals to community services
- Advocate for patient welfare and regulatory compliance
- Document assessments and progress in the EMR system
