Description : About the Role :

Touchstone is looking for a compassionate and experienced PRN Medical Social Worker (MSW) to support patients and families in their homes across San Antonio . In this dual Home Health and Hospice role, you’ll be part of a caring, highly collaborative team that values strong leadership, flexible scheduling, and the importance of work-life balance.

Key Responsibilities :

  • Provide in-home psychosocial support aligned with patients’ care plans.
  • Help patients and families navigate emotional, financial, and community resources.
  • Serve as a key liaison between patients, families, physicians, and the care team.
  • Attend patient care conferences and contribute to care planning.
  • Safeguard patient confidentiality and uphold HIPAA standards.
  • Assist with discharge planning and transitions of care.
  • Maintain accurate documentation and timely progress notes.
  • Participate in ongoing education and quality improvement efforts.

Requirements :

What We’re Looking For :

  • Master's degree in Social Work (MSW) from an accredited program.
  • At least 1 year of experience in a healthcare setting (home care or hospice preferred).
  • Excellent communication and relationship-building skills.
  • Ability to work independently and collaboratively across care teams.
  • Strong understanding of Medicare Conditions of Participation.
  • Current driver's license and reliable transportation.
  • Flexibility to support patients on a PRN / as-needed basis.
  • A deep commitment to patient-centered care and dignity at end-of-life.
