Job Description

We're seeking a compassionate and detail-oriented Hospice Registered Nurse Case Manager to join our dedicated team serving patients and families throughout Lancaster and York Counties . This role offers an opportunity to provide meaningful, end-of-life care that focuses on comfort, dignity, and quality of life.

The Hospice RN Case Manager is responsible for coordinating all aspects of patient care, performing comprehensive assessments, and collaborating closely with the interdisciplinary hospice team to deliver individualized, compassionate care in the patient's home or facility setting.
