Jet Health, Inc. is seeking a full-time Intake Coordinator to join our team at our corporate office in Fort Worth, TX .

The Intake Coordinator is responsible for the timely coordination of the referral process, including the review, response, and coordination of care for patients referred to Jet Health, Inc. This role requires strong communication and organizational skills, as well as specific experience verifying eligibility and benefits for home health services .

Requirements :

• Coordinate the referral process for patients referred to Jet Health, Inc.
• Monitor, review, and respond promptly to patient referrals.
• Verify and document insurance eligibility and benefits for home health services .
• Develop and maintain referral relationships within the community.
• Interact with facility representatives, discharge planners, and other agencies to facilitate care initiation.
• Conduct reverse marketing efforts in coordination with the Business Development Leader.
