About The Position

Responsibilities

• Evaluate, diagnose, and treat residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans tailored to each resident's needs and goals.
• Provide direct therapy to enhance communication abilities, cognitive function, and safe swallowing.
• Collaborate with interdisciplinary team members-including PTs, OTs, nurses, and physicians-to support holistic patient care.
• Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility and state requirements.
• Educate residents, caregivers, and staff on communication strategies, swallowing precautions, and home programs.
• Ensure all therapy services are delivered in compliance with applicable federal, state, and facility regulations.

Qualifications Current, active Oregon Speech-Language Pathologist (SLP) license required. Master's degree in Speech-Language Pathology from an accredited program. Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates may be considered. Experience in a skilled nursing facility (SNF) or similar rehabilitation environment preferred. Strong interpersonal, communication, and documentation skills. Compassionate and resident-focused approach to care.
