Position Summary
Join our team as a per diem Home Health Aide (HHA) ! Employment begins when you accept your first patient assignment, giving you flexibility while making a meaningful impact. You will provide personal care and support to patients in their homes under the guidance of our nursing staff.
Key Responsibilities
- Assist with personal care : bathing, dressing, grooming, toileting, and mobility.
- Support ADLs and IADLs : meal prep, light housekeeping, and errands as assigned.
- Take and record vital signs as directed.
- Observe and report changes in patient condition to supervising RN.
- Follow individualized care plans and complete documentation.
- Maintain a safe, compassionate, and dignified environment for patients.
