Kandu extends recovery beyond the hospital through principal illness navigation, providing one-on-one education, care coordination, and advocacy; grounded in clinical evidence and informed by the lived experiences of patients and their families.

This position provides initiating visits (evaluation & management) to stroke survivors and their care partners as an evaluation of patients for principal illness navigation.

What You’ll Do

• Perform a telemedicine evaluation visit to establish care and qualification for Principal Illness Navigation (PIN) for stroke survivors post-hospitalization including determination of :
• Whether the survivor has a high-risk condition expected to last at least 3 months that places the patient at significant risk of hospitalization, acute exacerbation, or functional decline
• Whether this condition requires the development, monitoring and / or revision of a disease-specific care plan with frequent adjustments to the treatment regimen
• Perform telemedicine visits to review stroke survivor qualifications for advanced rehabilitation technology and write device prescriptions, when appropriate.
• Prepare for visits by reading chart notes from the patient’s EMR and completing timely documentation for initiation of PIN.
• Communicate with patients asynchronously using the provider platform (as needed).
• Refer patients internally to PIN as well as other supportive services including behavioral health, dietician and externally to services such as PT, OT, SLP, etc.
• Coordinate and communicate care plans with external healthcare providers, as needed.
• Participate in weekly clinical meetings, educational sessions and broader team meetings as requested.
• Use and adapt to multiple technological platforms.
