TITLE : Family Support Specialist II
REPORT TO : Family Support Specialist Manager

JOB SUMMARY :
The Family Support Specialist II is responsible for providing comprehensive case management, referral, and seamless coordination with community and supportive service activities that promote economic self-sufficiency for families, enhance quality of life for seniors and the disabled, and increase opportunities for residents. This position will work with a team of social workers, case managers, relocation and employment specialists, and other professionals to identify appropriate strategies and assist families under Choice Neighborhoods Implementation (CNI) Grant from the U.S. Department of Housing and Urban Development.
JOB RESPONSIBLTIES :
- Conduct intake and assessments for resident enrollment in the case management program.
- Perform home, office, and community site visits to gather information from residents.
- Collaborate with families to assess, develop, implement, monitor, and modify Development Plans via a multidisciplinary process.
- Use knowledge of the local community to help residents navigate social services and public agencies.
- Identify service gaps, advocate for residents, and coordinate / monitor service delivery for an assigned caseload.
- Maintain privacy and confidentiality of all household information.
- Enter and track resident data in Urban’s LEARN performance management system to document progress in real time.
- Attend community supportive services meetings with key partners and other social service entities.
- Actively participate in Urban Strategies program meetings and other project-related meetings.
- Be available for occasional night and / or weekend work and travel as needed.
- Perform miscellaneous job-related duties as assigned.
QUALIFICATIONS :
Bachelor’s degree in Social Work, Sociology, Psychology, or a related field.Minimum three years of direct practice experience as a case manager, service connector, or with low-income households; health services experience a plus.Commitment to strengths-based, family-focused service planning and a place-based approach.Ability to handle privileged information professionally and confidentially.Strong team player with effective internal and external relationship skills.Availability to attend evening and weekend functions as required.Valid driver’s license and automobile available for work.Strong verbal and written communication skills.Proficiency in Microsoft Outlook, Word, and Excel.Strong interpersonal and communication skills; ability to engage a diverse community.Excellent organizational, planning, and time-management abilities.Commitment to confidentiality for participants and colleagues.Ability to thrive in a fast-paced environment.Ability to work across culturally diverse settings with cultural sensitivity.Ability to work independently with initiative and collaboratively in a team to achieve goals.Proficiency in MS Office Suite (advanced Excel) and other commonly used software.Urban Strategies is an equal opportunity employer
