Physical Therapist (PT) – Home Care | Contract-to-Hire

Location : Albuquerque, NM

Company : MedAdventures

Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM

Note : Local candidates only – no travelers . This is a contract-to-perm opportunity with intent to hire the clinician permanently.

Position Overview

MedAdventures is seeking a compassionate and experienced Physical Therapist (PT) for a home care role in Albuquerque, NM . This position is ideal for a local clinician seeking stability and long-term placement in a supportive home health setting.

You’ll provide in-home therapy services to elderly participants, collaborating closely with an interdisciplinary team to support mobility, safety, and functional independence.

Key Responsibilities

  • Evaluate participants for acute, restorative, or maintenance-level physical therapy needs
  • Develop and implement treatment plans in coordination with physician orders
  • Provide hands-on therapy including therapeutic exercise, mobility training, and modalities
  • Educate participants and caregivers on safe techniques, home exercises, and use of assistive devices
  • Communicate clearly with caregivers and team members to ensure continuity of care
  • Delegate appropriate tasks to PTAs and Rehab Aides based on care complexity
  • Maintain timely and accurate documentation of assessments, care plans, and outcomes
  • Attend interdisciplinary care meetings and contribute to plan-of-care discussions
  • Conduct home visits and assess safety, equipment, and environmental needs
  • Manage DME use and documentation
  • Support onboarding or training of new staff if needed

    • Required Qualifications

  • Bachelor’s Degree in Physical Therapy
  • Graduate of an accredited PT program
  • Active New Mexico PT license
  • Minimum 1 year of experience working with the elderly or frail in home care or a similar setting
  • Valid driver’s license, transportation, and auto insurance

    • Preferred Qualifications

  • 2+ years of experience as a practicing PT
  • MSPT or DPT degree
  • Bilingual in Spanish or another language is a plus
