Responsibilities :

Conduct patient admissions and deliver compassionate hospice care.

Complete same

day documentation using HCHB

Manage 20

25 cases weekly, achieving 26

28 productivity points.

Work 4 x 10

hour shifts with schedule flexibility. - No Weekends

Attend weekly in

person office meetings.

Qualifications :

Active RN License with 2+ years of hospice experience.

Proficient in HCHB

BLS certification required.

Strong independent work and time management skills.

Details :

No on

call or overtime.

Mileage reimbursement.

Flexible scrub color.

3 days of orientation and team introduction.
