About the Role :

Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? Callen-Lorde is seeking a Primary Care Provider (NP / MD / DO) who specializes in HIV care to join our collaborative, multidisciplinary team.

As a PCP at Callen-Lorde, you key responsibilities will include :

Clinical Care

Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.

Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines.

Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals.

Document all care promptly in the electronic health record (EPIC).

Provide primary care services

Collaboration & Coordination

Make appropriate internal and external referrals.

Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.

Quality & Professional Development

Participate in quality improvement initiatives and case conferences.

Engage in ongoing professional development opportunities, including continuing medical education.

Work Schedule :

Three morning / daytime shifts : 8 : 30 AM – 4 : 30 PM

Two evening shifts : 12 : 00 PM – 8 : 00 PM

Optional Saturdays : 9 : 00 AM – 3 : 00 PM

Compensation

The salary range for this position will be commensurate with the experience and qualifications of the selected candidate.

Additional differential of $10,000 annually for obtaining and maintaining Board Certifications in the areas of MDs and DO's - Infectious Disease, Endocrinology, Geriatrics, and Adolescence Medicine. This role(s) is a part of 1199 SEIU collective bargaining agreement

Qualifications

• New York State Certification / Licensure (MD, DO, or NP)
• Current NYS Registration and DEA Controlled Substances Registration Certificate
• Current CPR Certification
• Current AAHIV Certification
• Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease (for MDs / DOs)
• Experience in community-based primary care preferred
